Discharge Planning
Discharge planning begins early in every resident’s stay to support a safe, successful transition home or to the next level of care when appropriate.
Our dedicated social worker partners with residents, families, physicians, therapists, and nursing staff to coordinate discharge plans that promote long-term recovery and reduce the risk of hospital readmission.
Services may include:
- Home safety planning
- Home health coordination
- Equipment recommendations
- Caregiver education
- Community resource referrals
- Insurance assistance when appropriate
- Follow-up appointment coordination
- Ongoing support planning
